Ingress of moisture into the wall cavity
When water is allowed into a wall cavity it creates the perfect environment for mold, as seen by the off coloured insulation.

Asyou can see the, wall is only a 2" x 4" stud wall. This is insufficient for the amount of batt. insulation installed.
